The path forward is often clearer when we know where we are coming from. We ask that any relevant medical records be provided ahead of your first appointment. Our Support Specialist will reach out to guide you through this process.
Once we have received your medical records, our Support Specialist will reserve your space on the calendar and help you sign into our client care portal.
Attend regular meetings with your provider to ensure your care is current, relevant and fine-tuned!
