Case Manager
ph. 707-390-0134
Lara Celli is thrilled to serve as a case manager with Humboldt Counseling Connection. She is passionate about assisting clients in achieving physical, mental, emotional, social and spiritual health. Her ethos is compassionate care for all individuals and a belief in holistically approaching their unique and varied needs. She recognizes the importance of developing therapeutic relationships, sensitively evaluating individual and family needs, and case management with consideration of the unique backgrounds and experiences that deeply affect who we are, how we interact with the world, and obstacles to individual and family well-being. Together with her clients, she explores access to community resources, barriers to care, life stressors, addiction issues, personal relationships, spirituality, mental health and cultural and gender diversity.
Lara has worked as a social worker and a nurse in the Humboldt County community for three decades. She graduated with a Bachelor of Arts degree in both Social Work and Spanish from Cal Poly Humboldt. After graduation, she served as a social worker within a local elementary school setting, where she worked with children and families facing resource scarcity and sought to connect them to housing, food, clothing, employment, legal and academic assistance and other community resources. In this setting she additionally taught English as a Second Language to Hmong and Latino students. She earned her Registered Nursing degree at College of the Redwoods in 2007 and worked in the reproductive health care setting for over a decade. In 2018, she completed her Master’s Degree in Nursing through Frontier Nursing University in Kentucky. She then assumed the role of a primary care provider at Open Door Community Health Center. For the past three years, she has been employed by Planned Parenthood of Northern California, providing reproductive health care. Her roles as a social worker and nurse practitioner have allowed her to observe the unique aspects of the individual and family experience, participate in connecting people to fundamental and essential community resources and personalize care coordination and care plans with clients to promote optimal health and well-being.
Aside from work, Lara enjoys running and hiking, painting, dancing, reading and cherishing time with her children, Sylvia and Nico.
